Project title: INTEGRATION OF THE GENETIC, CLINICAL AND BIOLOGICAL PARTICULARITIES IN AN ASSISTED DECISIONAL INFORMATIC ALGORITHM FOR THE DIAGNOSIS AND TREATMENT OF OBESITY

Abstract:

Obesity is a major health problem, with numerous social and economic implications. It is associated with cardio-vascular, metabolic, respiratory and digestive diseases thus constituting a high risk clinical condition. Early identification of obese patients at high risk of failure to conventional therapeutic interventions and their reorientation towards alternative methods would reduce the time in which the patient is exposed to risk and would help to decrease the incidence of complications, to optimize cost-efficiency ratio and improve quality of life for patients. The working hypothesis is that: there is a genetic and proteomics profile that correlates with the prognosis and the treatment efficacy in obese patients with therapeutic interventions. 

Objectives: We propose to develop a protocol for the diagnosis and treatment of obesity, through an informatical system designed to facilitate optimal treatment choice. Parameters will be identified with prognostic value for the efficacy of different therapeutic interventions in obese patients. A secondary objective is to identify genetic factors with a pathogenic role in obesity (FTO gene mutations).

Material and methods: A lot of patients with morbid obesity and failure in conventional therapeutic interventions, proposed for Bariatric Surgery interventions and a lot of patients with good response to classic treatment will be evaluated. Investigations will track the characterization of clinical, psychological (dietary behaviour), hormonal (stress hormones, ghrelin, leptine, other adipocytokines), and molecular genetics (mRNA of ghreline expression in gastric mucosa, FTO gene mutations). The data acquired will be the basis for designing an informatical system to investigate the correlation between the clinical and biological factors, the molecular and genetic ones and the treatment efficacy, generating an optimized therapeutic algorithm. The final aim of the project will consist in building a structure for interdisciplinary reference research, which will include clinicians, physicians and surgeons specializing in obesity surgery, researchers as well as psychologists. The proposed information system will ensure interaction between these specialists and the development of a complex virtual environment, accessible to a broader category of physicians, who will manage, analyze, and evaluate medical data sets in order to assist in the decision-making process. This will help to reduce default medical errors and to increase the quality of healthcare.
